Benefits Summary

At the City of Farmers Branch, we are committed to providing a competitive benefits package to complement our strong compensation program.  The following summary provides an overview of the types of benefits offered to our employees.  More details are available from the Human Resources Department upon employment.

ELIGIBILITY As a full-time employee, you are eligible for many benefits on your first day of employment.  There is no waiting period for health benefits, holiday pay, deferred compensation, flexible spending accounts or the retirement plan.
COST TO YOU

The City subsidizes a large portion of medical insurance costs and provides basic insurances at no cost to you such as life insurance, accidental death and dismemberment insurance, and long-term disability insurance.

CHOICE

The City offers a cafeteria style benefit program where annually, you are provided a dollar amount per month to spend on the benefits you want.  This gives you the choice of spending your benefit dollars on those programs that best meet your needs.  Any remaining dollars go into the medical flexible spending account for you to use on out-of-pocket medical expenses.

TYPES OF HEALTH BENEFITS YOU MAY CHOOSE
MEDICAL

A consumer-driven medical insurance program through United Health Care's Choice Plus Network that includes four medical plan choices:  two Choice Plus Preferred Provider Organization (PPO) plans and two Health Savings Account (HSA) plans.  These plans give you maximum choice.  You may use the Choice Plus Network providers to maintain cost-effectiveness and maximize benefits; OR, you may see the doctor of your choice and pay an annual deductible and co-insurance.

The Health Savings Account medical plans are traditional, high-deductible health plans that allow you to set aside pre-taxed dollars into a Health Savings Account on a monthly basis to fund your health expenses both now and in the future.  If you elect one of these plans, the City will contribute up to $500 pro-rated over the course of a year into your Health Savings Account.  And, if you decide to add dependents on your medical plan, the City will match your contribution to a maximum of $250 - $500 into your Health Savings Account depending on the number of dependents covered under the medical insurance plan.

The Health Savings Accounts are built to secure funding for health expenses both now and in the future.  The account becomes your own, personal savings account which means that the money deposited into the account is solely yours to use and keep regardless of your employment status at the City of Farmers Branch.

DENTAL Three programs to choose from through Guardian:  Network Access Plan (PPO), Value Plan (PPO), and DHMO.  Network Access Plan allows you to utilize in- or out-of-network benefits, including a traditional indemnity dental plan where you go to any dentist, pay a $50 deductible and co-insurance.  Value Plan is similar to the Network Access Plan, but gives you more "value" in-network paying a higher amount toward in-network dental expenses.  Whereas, DHMO requires you to go to your primary care dentist from the network, obtain referrals to specialists, and pay a specified co-pay for dental services.
VISION                      Superior Vision provides a comprehensive vision plan for employees with in- and out-of-network options.  In-network providers require a $10 co-pay for an annual vision exam and $10 materials co-pay covers up to $100 for glasses and hardware, plus a 10%-30% discount on frames, or pays up to $120 for contact lenses annually.  Out-of-network benefits allow reimbursement up to $42 for an eye exam, $84 for lenses and $48 for frames or $100 for contact lenses annually.
CORE BENEFITS As a full-time employee, you are automatically enrolled in the City-paid Long Term Disability (60% of base annual salary), Accidental Death and Dismemberment Insurance and Life Insurance at 1 times your annual salary (minimum, $30,000).
ADDITIONAL COVERAGE
You also have the opportunity to purchase supplemental Life Insurance and Accidental Death and Dismemberment coverage for you and your family.  This is at reasonable group rates.  You may contribute to a medical or dependent day care flexible spending account to help pay for out-of-pocket medical and day care expenses with pre-tax dollars.  Supplemental insurance options provided by AFLAC are also available for Sickness, Intensive Care, Hospitalization, Cancer, Short-term Disability, etc.
